SKILLED NURSING (SN) EVALUATION AND VISITS (AS ORDERED BY YOUR PHYSICIAN)

• A Registered Nurse comes in for observation, assessment of condition and start of care.
• A Registered Nurse educates the patient and family of disease process, home safety etc.
• A Nurse follows up on subsequent visits to manage and evaluate the patient’s care plan, medication and effectiveness of home care as well as wound care, IV therapy/injections, diabetic care and education of patient and caregiver.

THERAPY SERVICES

Per the physician’s order the following therapeutic services will be provided

• Physical Therapy

• Medical Social Work

• Home Maker Services
• Home Maker Services
